A Fresh Perspective on Patient-Centered Care: Meet Jordan Smith, PA-C at Maple Health



At Maple Health, we believe that healthcare should feel personal, accessible, and truly patient-focused. That’s why we’re proud to introduce one of the newest members of our team: Jordan Smith, PA-C. With over a decade of experience and a passion for personalized medicine, Jordan brings both deep expertise and a fresh enthusiasm to our Direct Primary Care (DPC) model.


We sat down with Jordan to learn more about what brought him to Maple Health, what excites him about DPC, and how he hopes to impact the lives of his patients.



Why DPC? Why Now?

After more than 10 years working in various healthcare settings, Jordan has seen firsthand the frustrations patients and providers face in today’s traditional healthcare system. That’s what made Direct Primary Care so appealing.


“The DPC model resonated with me because it puts patient care first and seeks to overcome many of the system’s current failures,” Jordan explains. “At Maple Health, we’re supported in practicing medicine the way we’ve always wanted to — spending meaningful time with patients and focusing on what truly matters.”


Jordan was also excited to reunite with Dr. Berry and Courtney, two colleagues he’s worked with in the past. “Who you work with makes a huge difference,” he shared. “The opportunity to be part of this team again was incredibly exciting.”



First Impressions of the DPC Model

So far, Jordan’s experience with Direct Primary Care has been nothing short of empowering.


“For the first time, I’m not constrained by how long I can spend with a patient,” he says. “We’re able to truly delve into their concerns, build relationships, and get to know them as individuals.”


This ability to take time with patients is something many healthcare providers dream of — and now Jordan gets to live it daily.



Passion for Wellness, Rooted in Experience

With a background in exercise physiology and a lifelong love of sports, Jordan has a natural enthusiasm for health and wellness.


“Helping people take ownership of their wellness is one of the most rewarding parts of this role,” he says. “I love having the time to educate patients on their conditions and work together on strategies to improve their health.”


This hands-on, collaborative approach is what makes DPC so effective — and what patients at Maple Health have come to value.



A Return to the Roots of Primary Care

Jordan envisions building strong, lasting relationships with his patients — relationships that are often lost in the volume-driven world of traditional healthcare.


“The ability to know a patient, to know their normal, and to recognize when something is off is vital,” he shares. “I’m excited to return to those roots and practice medicine the way it was meant to be practiced.”

Jordan’s passion for learning and helping others shines through in everything he does. As he begins this new chapter at Maple Health, he’s filled with gratitude — and eager to welcome new patients.
